Basalt Health, an AI platform designed to help patients transition into post-acute care, has raised $20 million in Series A funding to expand its technology and healthcare operations.
The round was led by returning investor NEA, with participation from existing investors Frist Cressey Ventures and 25m Health. The latest financing brings Basalt Health’s total capital raised to approximately $24.5 million.

This funding gives us the power to transform one of healthcare’s most stubbornly inefficient systems.”
Founded in 2024, Basalt Health is focused on improving the post-acute care placement process which often involves healthcare teams reviewing large clinical referral documents and contacting multiple facilities to find an appropriate placement.
The company’s AI platform reviews referrals as they arrive through fax, healthcare portals or direct electronic medical record integrations. It evaluates patient information against facility clinical criteria and supports additional workflows such as admissions, benefits verification and prior authorizations.
Basalt Health has been working with Lifepoint Health since its seed round in 2025. The company says its technology has reduced median chart processing time from 8.5 minutes to 1.2 minutes. Basalt is also being implemented across 49 additional Lifepoint markets by the end of 2026.

The platform is expected to scale to 62 ScionHealth hospitals by the end of 2026, covering a network of more than 6,000 practitioners.
Basalt Health will use the new funding to expand beyond admissions into discharge and payer workflows and grow its sales, marketing, implementation and engineering teams.
